Circles of Support

The ‘I’ TEAM (Together Empowered And Motivated) – my life, my community is support for people to build their own ‘I’ TEAM’s made up of friends, family and associates.

This project has worked with 15 individuals/families across North Wales, supporting them to build a network beyond paid staff. Take a look at the video to the right too see how they have got on!
