Hubs
Services working in partnership with each other is really important to making sure that people get the right support, at the right time.
A number of areas are moving towards a ‘hub approach’ where people can visit and get information, advice and assistance from local authority and third sector staff.
Denbighshire Talking Points is a model that Denbighshire County Council have put in place to provide all Denbighshire residents with information, advice and assistance.
Joint working
Pooling funding together can be a great way to help services work together better. Sometimes this can be tricky, where organisations have different ways of working. But this just means it’s even more important! To help services put pooled funds in place the Welsh Government gave some money to an organisation called the Association of Directors of Social Services Wales (ADSS Cymru). They worked with the Welsh Government, health boards and local authorities to write practical advice on how to go about pooling funds.
The advice is very helpful and points out that the reason for pooling funds is to improve outcomes for lots of different people, including people with learning disabilities. By pooling funds, services are able to safely and legally work better together to meet the health and social care support needs of individuals. This is especially important for individuals who need care and support from lots of different services and professionals. It also helps individuals get the care and support they need more quickly and this can help people to continue to have a good quality of life in their own home.

This year we are going to be supporting some pooled budgets around pooling funding between health and social care.
