Person-Centred Approaches

The North Wales Learning Disability Strategy tells us that we must be person-centred in everything that we do.

Being person-centred means enabling the person with a learning disability to be at the centre of their own care and support planning, as the expert in their own experience, and tailoring support to fit the person, not the other way around.

As person-centred approaches are all about helping people get the right support for them, you will also find resources and support that reflects a person-centred approach throughout the other sections within ‘The Right Support’.

We have a regional PCP plan that was coproduced with people with learning disabilities, their carers, providers and services.

Person-centred thinking tools

Person-centred thinking tools have been developed by Helen Sanderson Associates (HSA) and have been used successfully over many years to support people to think about and plan for their life in a meaningful way.

The person-centred thinking tools are a set of easy-to-use templates that are used to give structure to conversations. Using them is a practical way to capture information that feeds into person-centered planning, as well as to improve understanding, communication and relationships.
