Transition
Working Together to Improve Transitions into Adulthood
North Wales Together has been asked by the Regional Partnership Board to lead work on Transition Protocols across North Wales. The aim is to ensure that young people with learning disabilities experience a smooth and supported transition into adulthood.
This work involves:
- Collecting and comparing transition protocols from across the region
- Mapping similarities and gaps to ensure all key areas are covered
- Collaborating with families to make sure the protocols are clear, accessible, and user-friendly
We are currently supporting several Local Authorities in North Wales to update their Transition Protocols. A Steering Group will be established once this initial work is complete to guide the next steps.
Conwy Connect Family Transition Project
Supporting Young People and Families Through Life Transitions
Since 2020, Conwy Connect has been delivering the Family Transition Project, initially as a pilot and now available across North Wales. This service supports young people aged 14–25 and their families as they move from children’s to adult services.
The project offers:
- A single point of access to transition information via their Information Hub
- One-to-one, person-centred support
- Community groups and social networking opportunities for families and young people
- The goal is to make transitions smoother, more informed, and better connected.
