The North Wales Together programme was asked to complete this regional transition resource by the Learning Disability Partnership Group, Service Managers across health and social care in North Wales.
Transition means the move from children’s to adult services for young people with a learning disability.
This resource has been designed to provide young people, families and professionals in North Wales with clear and thorough information about transition, all in one place.
What You Will Find:
A set of Regional Core Principles, which tell you what should happen and at what age across the region, within health and local authorities.
Tabs for your local authority and for health. If you click on a tab, you will find the transition information for health and social services learning disability services in your area.
The Useful Information section provides information on a range of topics that are important to consider during transition from childhood to adulthood.
